Why partner with STRIDE Community Health?
- Stride Community Health has provided immunizations, child wellness visits, and flu shot clinics at sites across the district since 2018.
- STRIDE played a key role in the district’s timely response to the COVID-19 pandemic by providing COVID testing and vaccines throughout our communities.
- Through the SBCHCs located in CCSD, STRIDE will provide medical staff, supplies, health education, and counseling for students and families.

Who is STRIDE Community Health?
- STRIDE is a nonprofit community health center with a long history of offering affordable, high-quality, culturally-sensitive health services, such as primary care, dental, and behavioral health to all in the community.
- STRIDE welcomes people of all ages, races, genders, and cultures regardless of citizenship or insurance status.

Who can access the SBCHCs?
- CCSD students, families, staff, as well as community members, can access these STRIDE SBCHCs.
- Patients will be seen by appointment.

How to schedule an appointment with STRIDE?
- For appointments at the Overland High School or Horizon Community Middle School SBCHCs: call 720-554-4584
- For questions or to schedule an appointment at any other STRIDE location, please call: 303-7STRIDE (303-778-7433)
- New patients should request a Financial Screening Appointment so STRIDE can determine if they are eligible for Medicaid, STRIDE's sliding-fee discount program, or other grant-funded programs.

What insurance does the SBCHC accept?
- Accepted insurance: Medicaid/CHP+ and most private insurance.
- Check with your insurance provider to confirm if STRIDE is in-network.
- Unable to bill Kaiser, Denver Health, or Denver Health Elevate insurance.
- Uninsured? Call STRIDE at 303-360-6276 to schedule a financial screening appointment.
