Immunizations and Exemptions  We follow the Colorado Department of Public Health and Environment (CDPHE) guidelines regarding immunizations. You can email or fax your student’s updated immunization records to us at the clinic. If you choose to exempt your student from any immunizations, a new exemption form is required each school year unless a medical exemption is completed by your healthcare provider.
