Transition Program at Fremont
The Transition Program is an optional program for young adults with special needs who are 18-21 years of age and who have completed their high school core classes and when the IEP team have determined the need for such transition services.
Transition is the process students, and their families use to develop desired outcomes after high school. Transition assists with developing necessary tools to progress from high school to adult life.
Transition may look quite different from individual to individual. Services are provided through a "team model" approach of educational professionals, students, parents, and community resources.
Experiences and opportunities are offered to ensure that students can obtain individual goals and acquire the knowledge and skills needed to access appropriate community resources and agencies.
