Parent Forms Support

Go to my.cherrycreekschools.org and log in using your email address and password

Forgot your password?

If you forgot your Parent Portal password, enter your email address, click "Go," then select the "Forgot password?" link to reset it.

• Select the PowerSchool/Parent Forms tile
  This will open PowerSchool

• Once in PowerSchool, you’ll see the name(s) of your student(s) at the top
• Click on your student's name
• Click the Parent Forms button in the bottom of the left menu

• Enter your student's date of birth in the Date of Birth Authentication window. Use the date format MM/DD/YYYY

Tip: You can select a language by clicking the globe icon in the top right.
Parent Forms is available in Chinese, Korean, Russian, Spanish, and Vietnamese.

Tip: Your information saves automatically, so you can return anytime before submitting. Parent Forms must be completed using the same email address.

• Read the Introduction page, then click Next to continue

   

• Enter information for all required fields.
  Note: Some fields in grey boxes may be pre-populated with existing information and cannot be changed.

Once you have entered information for all required fields, verify on the Summary page you have all 🟢 green check marks.

Missing or incorrect information will have a 🔴 red bubble with the number of incorrect fields. Click the 🔴 red bubble to enter missing or incorrect information.

When all items have a 🟢 green checkmark, click Submit.

If you have more than one student, repeat this process for each.

Once Parent Forms have been submitted for a student, you are not able to make any updates through Parent Forms. To update information, log into my.cherrycreekschools.org and click the Guardian Update tile. Changes can be made 24 hours after submitting Parent Forms.

On the Guardian Update screen, you can update:

• Additional Contact Information
• Parent/Guardian Email Address
• Parent/Guardian Phone Numbers
• Parent/Guardian Spoken Language
• Parent/Guardian Communication Language (for district communications and meetings)
• Over-the-Counter Medication
• Automated Message Preferences
• Home and/or Mailing Address (changes will be reviewed by the Admissions department)
