STRIDE School-Based Community Health Clinics

CCSD has partnered with STRIDE Community Health to establish three standalone, school-based community health centers (SBCHCs) at Horizon Community Middle School, Overland High/Prairie Middle school campus, and Smoky Hill High School. These STRIDE SBCHCs will offer students, their families, the community, and staff more comprehensive and equitable primary clinic healthcare options.

- 2-day sliding scale co-pay collected before visit
Payment
- If a student presents to any SBHC and is Medicaid eligible, STRIDE will bill Medicaid.
- Uninsured patients, including students who come to the SBHC alone, can complete a one-day application for the Sliding Fee Discount Program.
- Students applying on their own will use only their personal income. If their annual income from work is under $31,300, they qualify for a discounted copay.
- Patients who don’t qualify based on family size and income will be asked to pay a $120 deposit at the time of service.
